RPM is a comprehensive health monitoring system that enables you to stay connected with InstaClinic® from the comfort of your own home. InstaClinic® teams monitor your vital signs to identify abnormalities or negative trends. Monthly interactions with our team and early detection of changes in vital signs provide synergistic prevention tactics.

Medical Weight Management Referrals
Obesity remains one of the most significant contributors to chronic disease progression.
We accept referrals for patients who may benefit from structured, physician-supervised weight management using compounded semaglutide or tirzepatide when clinically appropriate.

InstaClinic® was founded by Amy Surdam, Nurse Practitioner, and Daniel Surdam, MD.
We created InstaClinic® to provide structured, longitudinal medical care delivered virtually — without sacrificing physician oversight or continuity. Our model is designed for patients who benefit from ongoing management of chronic conditions as well as episodic concerns appropriate for virtual evaluation.
Care is delivered through a transparent monthly membership that supports consistency, follow-up, and direct access to licensed clinicians.

Amy Surdam is a board-certified Nurse Practitioner with experience in primary and emergency care. Her clinical focus includes medical weight management, hormone therapy, and chronic condition support.
